What the form collects

The assessment request form asks for your name and email (required), and optionally your practice name, phone number, city or region in Ontario, the challenge you are facing and anything else you choose to write. There is also a hidden field that only automated spam fills in; a real person never sees it.

Why

To reply to you about a practice assessment: to confirm scope, price and a date, and to prepare for the day. That is the only use. Your details are not added to a newsletter or marketing list, are not sold, and are not shared with anyone outside Jwan Practice Solutions, except the form and email providers that deliver the message to us.

How long it is kept

If we do not go ahead with an assessment, your submission is deleted within twelve months of your last message. If we do work together, your contact details are kept for as long as needed to deliver the assessment and the plan, and for the records a business is required to keep afterwards.

No patient data

Please do not put patient names, chart numbers or any patient health information in the form. The website does not need it and is not built to hold it. Anything an assessment requires from your practice-management software is reviewed on site or through a channel we agree on in advance, under a written confidentiality undertaking.

Cookies and analytics

This site sets no cookies of its own and runs no analytics or advertising scripts. Fonts are loaded from Google Fonts, which means your browser requests font files from Google's servers when the page loads. The site is hosted on Vercel, whose servers keep standard access logs for security and uptime.

Your choices

You can ask at any time what we hold about you, ask for it to be corrected, or ask for it to be deleted. Write to info@jwanpracticesolutions.com and we will reply within one business day.
